Please ensure the forms are completed and signed. On receipt of your completed Admission Form, the hospital will verify your level of private health fund cover. It is advisable for you to also contact your health fund to confirm your eligibility to claim.  To obtain a cost estimate, please contact Patient Accounts on (08)9367 0266 during office hours.  For further information see Fees and Charges. If you have chosen to fax or email your Admission Form please bring the original form in with you on your day of admission.

If you’re being admitted for an overnight stay or staying longer with us, we have a range of private and shared rooms available.

All our rooms have the following facilities:

Day of Admission

Your doctor will notify you of the time you are to present to be admitted.  Admission time is not decided by the hospital.

Please follow carefully all pre-operative instructions given to you by your doctor.  Failure to do so may result in your procedure being delayed or cancelled.  Check with your doctor if you should take your regular medication on the morning of the operation/procedure.
